Cooperation

Planning and diagnostics. We will support you as early as during the planning stages of treatment for patients that you feel may need implants or prostheses. If you so wish, this can also be done at your practice. You are more than welcome to benefit from the diagnostic possibilities of modern three-dimensional digital volume tomography at any time.

Treatment. We will handle referrals for augmentation surgery and implantological surgery for you. Immediately after treatment has been completed we will report back to you by phone and/or in writing. You will also receive X-rays from before and after the surgery.

Transfer of information. Following referral of a patient for DVT, you will receive our diagnostic in writing and, if applicable, information on planning. Alternatively, you may also only be sent the DICOM data stock for your use.

 

We provide accurate diagnosis for tooth preservation, even when the teeth have already been treated repeatedly for preservation and restoration, or the description from patient is not clear enough for diagnosis. Occlusal, periodontal or non-tooth-related causes are often difficult and time-consuming to diagnose. We are here to help you.

Cooperation

  • Before the treatment begins, we offer all our patients an individual consultation for clarification and open questions. The patients should always be well informed in terms of their own share of the total treatment cost.
  • After the treatment with us, the patient is referred back to you together with our treatment report.

Referral form for endodontic treatment

 

Prosthetic with adhesive bridges offers many advantages, especially for adolescent patients.

Advantages of adhesive prosthetic

  • Low level of invasiveness
  • No irritation for pulp
  • No local anesthesia needed
  • Periodontal prophylaxis by fixed restoration
  • Maintenance of conventional preventive alternatives

Advantages for patients

  • Suitable for adolescents for the growing of jaw finishes
  • Unnoticed removal of an adhesive casement if not possible (lower risk of caries)
  • Neighboring teeth are not blocked
  • Simplified preparation

Treatment

In principle, we recommend all our patients to have an individual consultation before any treatment.

If possible then the debonding of the multi-bond device should not take place before this appointment.

Possible indications for adhesive prosthetic

  • Single tooth gaps, especially non-aligned.
  • Width of the gap should correspond to the width of the tooth to be replaced.
  • Neighboring teeth are caries free (small fillings are acceptable)
  • Application after the growth of canine teeth and horizontal jaw
  • 8 weeks of waiting time after the active movement of the tooth has been completed

Information about referral

Each tooth that is worth preserving for you is treated by us, as long as endodontically feasible. We are glad to help you with difficult endodontic cases. A good service for you and your patient is our top priority. 

When and how can you refer a patient?

Would you like to carry out the treatment yourself?

We are happy to offer you assistance on the phone or in writing, in case you would like to perform the treatment yourself. All we need is an X-ray picture, which you can simply send via email to info(at)hardermehl.de.

Referral for a consultation

In case of complex diagnosis or if the long-term prognosis of teeth is hard to estimate, we are also happy to assist.  

Referral for a treatment  

We would like to offer you help for any endodontic treatment that you do not wish to perform yourself. In addition to questions about root canal treatment, we also support you with information about implants and concerns in the field of orthodontics.

We are glad to receive patients from you for any endodontic treatment and therapy. 

However, please take note of the following:

1. In general, we recommend all our patients to have an individual consultation before any treatment begins. During the consultation, all information regarding the treatment is clarified in detail. In exceptional cases (e.g. long travelling distance), current X-ray pictures are used for the estimation of further treatment options. 

2. It would be very helpful if you can send the previous transmission (via email) of X-ray images. A short letter or telephone call from you would also be of great help to us.

3. The treatments could be time-consuming, and are usually spread over 1 to 2 sessions, total period of 2 to 4 hours.  We will take the time necessary to ensure the success of the treatment. Therefore, the invoice for insured patients is to be settled according to GOZ. The patient must be well informed by you regarding their estimated share of cost.

4. Even for privately insured patient, certain cost may not be covered by insurance, depending on the difficulty level of the treatment. The patient shall also be well informed in advance.

5. We could also finish the treatment with a dentin tooth structure at your request.

6. After the treatment is completed, a treatment report is produced and sent back to the original dentist together with the patient.

Endodontic Assistance

Extra caution is advised in following situations:

A precise diagnosis can be very difficult when the symptoms are vague. Standard tests sometimes produce contradictory results and the X-ray image may also not give any clear evidence.

Access to dental
If the patient has a limited oral aperture, it could be difficult to treat any posterior teeth.

Sclerosed channels
Often with elderly patients, or patients with restored teeth or teeth trauma, sclerosed channels can be difficult to locate.

Curved channels
Curved channels are often difficult to treat and may require special techniques for a clean preparation. Without special instrumentation stepping within the canal can be caused or even in the worst case perforation could occur.

Revisions
If the previous root canal treatment fails, then it is best for this to be revised by an expert. This includes the dissolution of blocked canals, the closure of perforations, the removal of pins and root canal filling materials of all kinds, and of course, broken instruments and silver points.

Root-tip resections
In the past, many failed root fillings were treated with root-tip resections. With this therapy, the chance of success is only about 50%. The orthograde revision of inadequate root filling and, on the other hand, the reduction of germs results in a much better chance of success. Although with the help of the modern techniques, the need for root-tip resection is much lower now, this therapy is sometimes still required. In such a case of necessity, we make use of our most up-to-date microsurgical techniques.

Root canal treatments as an elective procedure
In certain situations, the pulp must be sacrificed due to restorative reasons or questionable vitality. Root canal treatment as elective treatment may also be indicated even if the tooth has no symptoms. However a root canal treatment can be indicated as an elective procedure, even if the tooth shows no symptoms, but the preparation for the crown or a renewed preparation of the coronal seal has caused injury and could lead to an infection.
